    Who taught you the most ever and what did they teach you?

Student's email address, other activities they're going to be involved in this year, would you describe yourself as a successful Math student & why or why not, what do you hope to get out of this course, do you prefer to work by yourself or with other people, what do you see yourself doing as an adult, in which of the following areas do you do best (classwork, homework, projects, tests)

    "I learn best when..."

    I second Anna's idea to have them say whether they like working alone or with others best.

    Have you considered having kids complete a learning style inventory? If so, it would be good to have their primary learning style listed on the student survey for quick reference.
